Common OPD Procedures

Laser treatment

Laser surgery is an outpatient procedure. Some of the conditions that laser surgery is indicated in are as follows:

  • Diabetic retinopathy
  • Retinal vein occlusion
  • Retinal tear
  • Macular degeneration
  • Retinopathy of Prematurity

Laser ((Light Amplification by Stimulated Emission of Radiation) works by using a light beam to cauterize (seal) leaking blood vessels, or retinal holes and tears. It may involve placement of a lens on the eye. However there is minimal pain, and the patients may return to work the next day in most situations.

PASCAL  (Pattern Scan Laser):-

Narayana Nethralaya boasts of an advanced version of laser photocoagulation system.
The PASCAL  (Pattern Scan Laser) Photocoagulator is a new system designed to treat retinal diseases.
There are several benefits with the Pascal laser compared to the conventional laser. Most of the laser can be done in one sitting, unlike the conventional laser, which requires two to three sessions. This in turn reduces the number of patient-visits to the hospital. The procedure also significantly reduces the discomfort associated with conventional laser, and therefore patient tolerance is much better.
Pascal laser can be used in most situations where conventional laser is indicated.

Constellation Vision System:-

The vitreo retinal surgery has seen tremendous improvement in techniques and instrumentation in recent times. The availability of sutureless vitrectomy has added a new dimension in this regard; this procedure coupled with high speed cutters makes the whole procedure safer and more predictable. The recent launch of Alcon’s Constellation vision system provides never imagined features like. (1) High performance 5000 CPM (Cuts/minute), which allows the surgeon to work  closer to the retina in a safe manner. These high speed cutters cause less iatrogenic complications. (2) Surgeon controlled duty cycle, this makes for more precise control flow(less turbulence and less movement of retina). (3) Integrated pressurized infusion and Intra ocular pressure control. (4) Advanced Xenon illumination, allows better visualization. (5) Embedded 532 nm PUREPOINT Laser.
Suture less vitrectomy that is possible with Alcon’s Constellation vision system has similar kinds of benefits as of phacoemulsification in cataract surgery. (1) Lesser surgical time thus ensuring less inflammation. (2) Greater comfort during the post operative period. (3)Faster recovery time and less post operative astigmatism. (4) Less tear film instability. (5) quicker and comfortable healing. All instruments including cassette, cutter, light pipe, trocars and plugs, infusion line are available in a sterile pack, which can be disposed after a single use. This ensures that the patient gets the best of technology with the highest standard of safety and sterility.

Photodynamic therapy

Photodynamic therapy is a specialised form of laser treatment used for patients with age related macular degeneration (AMD). A light- sensitive drug is injected into a vein and travels to the abnormal blood vessels in the macula. This is activated by the laser. The light-activated drug then destroys the abnormal vessels.

Cryotherapy

Cryotherapy is a second way of treating retinal tears apart from laser. An extremely cold probe is used to "freeze-burn" a small area on the outside of the eyeball that overlies the retinal tears. The purpose is to seal the tears and create an eventual scar that will "stick" the retina to that spot.

Intravitreal injections

Drugs are injected into the vitreous cavity for various conditions like ocular infection, severe ocular inflammation, vitreous hemorrhage or severe macular edema in diabetics. These could be antibiotics, anti VEGF agents (those which reduce the new blood vessels in the eye) and steroids.

Pneumoretinopexy

Pneumoretinopexy is a method of treating selected cases of retinal detachments. A gas bubble is injected in the eye after applying cryo spots to the area of retinal tear. The patient is expected to maintain a certain posture after the procedure for about a week to 10 days. Also, he is not allowed to travel by air during this period.

Anterior Chamber / Vitreous tap

A very small amount of fluid from inside the eye is removed in cases of suspected infection in the eye or persistent inflammation. This fluid is then analyzed microbiologically and biochemically to aid in the diagnosis.
